BuckIPE Student Feedback Survey

The BuckIPE Student Feedback Survey is completed by students at the end of each BuckIPE experience.  The results of the survey are used by the Office of IPE and BuckIPE Faculty Champions to improve interprofessional learning experiences.  The survey includes demographic questions, questions regarding their instructor(s)/facilitator(s), perceptions of effectiveness, relevance, and other content specific areas, as well as student feedback on facilitators and barriers to learning.  This survey also gathers student nominations for facilitator awards.

Questions include:

1. For this BuckIPE experience, please rank the extent to which you agree with the following statement: (5-point scale, strongly disagree to strongly agree)

• The facilitator had a positive impact on our team discussion and learning experience.
• (If strongly or somewhat disagree is chosen) Comment on what would have improved your facilitator experience (open-ended)

2. Please rate your level of agreement with the following statements regarding your BuckIPE Instructor(s)/Facilitator(s). (5-point scale, strongly disagree to strongly agree)

• The instructor(s) embodies the spirit of interprofessional collaboration through role modelling positive teamwork, sharing their own team experiences, and showing respect for all professions, etc.           
• The instructor(s) fosters student-directed interprofessional learning and leadership.
• The instructor(s) openly facilitates participants' learning from other team members' perspectives.   
• The instructor(s) encourages all participants to contribute to discussions and decisions.

3. The BuckIPE Facilitator of the Year Award recognizes the contributions of a facilitator in any setting whose commitment to the BuckIPE curriculum and core competencies positively impact learners at any stage in their IPE growth trajectory. If you feel your facilitator provided an exceptional experience for you, please enter their name below for consideration for this award and a sentence or two explaining why you nominated them.
